Monthly minimum usage fee of $199 – customer will be responsible for the greater of (i) Minimum Monthly spend or (ii) the actual incurred Usage Fees

-  Usage fee - customer will pay Company a fee of 3% of the total order value amount for orders where a claim is filed with the insurance carrier following purchase.
- Customer will be invoiced monthly in arrears.
- Company will bear no liability to Customer chargebacks.
